Speaking of which: I'm working from the PDF found on Archive.org (thanks 
Adam). The actual book is more than $100 online! That is nutty. I will 
soon be looking for helpers to sort through VCF's dozens of disorganized 
book boxes. :)  Hopefully we own a copy. I could print the PDF but it's 
not the same.
